Any PHP Gurus on board?

Hi all,

I need some help with a form I'm creating DW. It's a simple vote for A, B, C, or D with radio buttons and then a submit button. I've had a friend help me create a PHP script to collect the voting information and e-mail it to me.

It's all working fine, but we've balled it up somewhere because the results aren't showing up in the body of the e-mail like they should. Si, I guess technically it's not working.

If some kind soul out there wouldn't mind taking a look at the script, I sure would appreciate it. I think the problem is pretty simple to fix, I just learning PHP.
