Does anyone know of a .php based script that will enable a urfer to do the following on one of my sites:
Fill Out Their Personal Info
Submit And Image
All of this should be stored on our server preferably on a MySQL DB indefinately but we also need to be able to access all of the form details via the web includng viewing the submitted images.
