PHP Feedback Form With Image Attachment?

Does anyone know of a php form script that will allow the person using it to attach an image on to the form in addition to text and either email it or, upload the image to the server?

This one is free and nearly idiot proof (I was able to install and get it working, and you can put my knowledge of PHP in a thimble.)

It does support image upload, and will write either to a flat-file database that it creates, or to a MySQL table. You can set the maximum file upload size. We used to use it for our model recruiting link off of Boyfunk until our programmer redid it with full MySQL search/sort capabilities.
