How do you create a online model application form

It won't be as pretty as the Falcon form, but you can make a simple application form that saves either to a flat-file table or to a MySQL database using this application, which is free.

You can also make the basic form and have someone "pretty it up" for you.

It can be as easy or as complicated as you want to make it really.

The best thing to do is come with a list of needs, a list wants and list of what you don't want the form to have.

Then either post it here or on a php programmer auction forum and you could get it as cheap as probably $10 or as much as $100.00 depending on the features you want.
