What, exactly, makes a domain name a "premium domain name"?
I figure it's a domain that 1. uses a specific niche keyword and 2. usually one or two words at the most
Or are there other things to consider?

I'm still waiting for "gay.com" to become available! - LOL

we own www.gaylove.com and www.gaylovers.com
Premium or not?
Hmm -- good question. I'd have to see some stats on the usage of "love" and "lovers" as a search term. Have you developed these, or have plans for them? I'm getting images of (BF) self pic boyfriend type site(s).
We've got them on WL for now. Our portfolio contains a lot of what we consider premium domains in both the gay and straight markets.
"Premium" just means something someone will pay good money for. Until someone pays the money the premium label is a little theoretical.
In December I payed a pretty substantial amount for BBBH.com, but it's sort of a community established brand. The term has almost as much search traffic as 'rawtop', and is a pretty popular hashtag on Twitter, plus being 4 characters long, it's very mobile-friendly. To me that made it a premium domain that was worth paying for. Now I just have to get the programming done for the hookup site I want to put on it…
rawTOP: Sex Blog, Porn Blog, Forum, Tube, VOD/DTO Paysite, Twitter, Tumblr
Other: Wily Willy, Twisted Pigs, Hot Naked Muscle, Twitter
Correct of course rawTOP unless it's so obviously a premium name such as blowjobs.com or fetishes.com (for example).
BBBH.com looks promising. Congrats on the acquisition.
i believe a missed requirement would be that it is registered, with history, and 'just coming back to the market'
