    Suggestions on model release forms

    I am looking for some good model release forms that are up to date with all the current requirements.

    in shooting content, the general rule of thumb is you can never be too careful. two forms of id is best, and it can't hurt to have a witness at the signing, but i don't know of anyone who does this.

    A birth certificate is EASILY faked--notoriously fake are Texas City birth certificates--such as a birth certificate from the City of Laredo, Texas

    Tracy Lords had a fake ID when she performed in all of her [unbeknownst to the makers] underage films
    Asking for 2 forms of government ID is best--preferably a state driver's license and passport. I know that few 18 and 19 year olds have passports, so the best evidence is rarely available. And how many people carry a copy of their birth certificate around? If you do accept a birth certificate, it should be a recently issued certified copy from the county recorder's office of the county the person was born--not issued from a hospital. Even a novice can make a hospital birth certificate at home.
    Most 18 and 19 year olds will be able to show you a bank statement, a utility bill, something that shows that they have attained the legal age of 18 and can now be legally responsible for bills. If they give you a student ID, go to that college's website and see if they are registered there. I know of a particular case where a young man claimed to have played on the hockey team at the U of Winnepeg--a team that happens to have a nice website and surprise! A complete roster and team photos. He was nowhere to be found and he was nowhere near 18.

    I required a slightly modified version of what the state DMV requires to get your first drivers license (which was of course required in itself).

    Search google for "STATE dmv" and look around at what some of them require. It's pretty solid, and stuff everyone should have on hand if they know they are modeling ahead of time. Some states are more lax than others. Some do a points value for each ID. Others require one from each of a set of groups.

    Now if it comes out that one or more documents were forged, you can tell them your methods of IDing models is the same method they use to keep terrorists from getting drivers licenses. Nobody is going to argue with your honest attempt.

    What gets me are these moron webmasters who take possession of Eastern Euopean content where the only ID provided has no security features at all and could be reproduced on any printer made in the past 15 years.

    The best advice i can give you is to speak to a proper lawyer, Chad is good at what he does so get with him, youll be glad you did if anything ever needs to be proven in court or, to the authorities

    I videotape the guy signing the document and holding his ID.

    Models will try to give you anything and everything. I require a valid state issued ID/Drivers License with a photo or a valid passport. International passports can get tricky, so beware.

    Have an attorney draft you a model release form specific to your state. Nothing else will suffice.
