This one is free and nearly idiot proof (I was able to install and get it working, and you can put my knowledge of PHP in a thimble.)

It does support image upload, and will write either to a flat-file database that it creates, or to a MySQL table. You can set the maximum file upload size. We used to use it for our model recruiting link off of Boyfunk until our programmer redid it with full MySQL search/sort capabilities.
