There are no rules what a 2257 page should look like, only a rule that gives the exact language of what the LINK to the page must say.

The information on your 2257 page needs to include the name of the individual who is responsible for the 2257 database and the physical address where the individual and records are located. The address does need to be a place of business and it must not be a PO box. If that place of business is a home address where the business is located, then that is the address that should be provided. If the business has more than one location, the location where the records are stored is the appropriate location. The individual needs to be a real person that is an employee or officer of your company.


How the page looks is up to you, as long as the required information is on it then you have complied with the law as it stands today.

If anyone knows of any reason why this is incorrect, please PLEASE say so. As earlier stated, what I say is only an opinion and I have been wrong before--I think that is why it is called the PRACTICE of law :-) .
