Actually, content producers and site owners care a lot, as you can see from various piracy lawsuits going on. Its just that hotfile is exactly the same as rapidshare and other file sharing sites, they hide behind the safe harbour laws protecting legit hosting companies (even though everyone knows that 99.9% of their content is illegal file sharing). These are not new by any means, but unfortunately, the only effective solution at the moment is to send these companies DCMA notices every day to get content taken down again and again.
However, there are some interesting lawsuits under way against the uploaders of this content (see this xbiz article) which may make it easier to stop this action in the future.
IMHO, I think the adult industry needs to publicly attack PAYPAY for allowing these sites to use them as a payment system for illegal file sharing. If we get something in the papers that paypal is being used to pay for illegal porn, its sure to kick up a fuss and eventually get their paypal accounts shut down. Cut off the funding and you will see action.
