That's because they don't.

I've always felt uncomfortable selling something that is basically totally fraudulent. And I think it's pretty much proven that pills for penis enlargement are BS.

The sad thing is, the chargeback/return rate is probably very close to zero, because what guy who was insecure enough to buy the shit in the first place is going to call up and admit on the phone that his small dick is still small and that this marvelous product, which the manufacturer assures you works for everyone, didn't work for him and his dick is still small? To me, it's just wrong.

I do think the idea of selling pumps (along with sensible instructions about not overdoing it) is a good one, but I haven't seen anyone offering such a program on an affilate basis.
